An exciting opportunity has arisen within the Older Adult Mental Health Community services, where we are looking to recruit band 6 CPN to the North Community Hub.

The post holder will care co-ordinate a caseload and complete assessments of patients with complex mental health problems, within Older Adult Services. They will work along side and within the wider multi-disciplinary team.

You will find this post very rewarding working with older people who are suffering mental health issues including dementia. You will be part of a supportive, well established team who always go the extra mile for service users and carers.

You must be able to work independently and have the ability to prioritise and effectively manage the workload. The role requires close working with other members of our team, outside teams/agencies, and with our service users and families. The applicant will therefore need to be able to communicate effectively, and have the skill's to communicate those who may be more difficult to engage.

You will be an advocate for your service users and be a strong team player.

The applicant must have a flexible and 'can do' approach, with the ability to work under pressure and adapt to changes, often at short notice. A positive attitude is essential for effective team work and management of the challenges, rewards and complexities within our work.
