Key Requirements:

A good standard of general education, including Maths and English.
Educated to Level 3 standard (such as NVQ) in a relevant subject.
Experience of working within a health and social care environment.
Experience of building positive relations with a wide range of social care and health professionals, members of the public and people who use services and their families.
Experience of looking at ways to improve working practice and procedures.
Experienced in the use of computer systems and databases.
Experience of working within a multi-disciplinary team.

The role of Hospital Discharge Coordinator is to support operational social care teams with ensuring people return home from hospital in a safe and timely manner, through successful partnership working with Health and Social Care. The role will take a proactive coordination approach covering all discharge pathways.

Key Responsibilities:

Be responsible for the coordination and management of a daily position statement and circulated daily, or as and when required in times of surge, to all partners.
To be able to confidently and professionally challenge decisions, delays and queries which impact the flow of hospital discharge.
To participate in effective solution focussed discussions to manage flow. Escalating to the Hospital Discharge Manager where there are significant delays or risks.
Ensure there is access to care and support if needed on the day of discharge and work with partners to reduce delays and promote wellbeing.
To work closely with health, housing, and voluntary sector colleagues to ensure resources are used effectively across the system. Multi-disciplinary working will be critical to the success of this role.
To work in accordance with safeguarding policies, safeguarding individuals in a way that supports them in making choices and having control. Ensuring that the ‘Making Safeguarding Personal’ ethos is at the heart of all practice.
To develop an awareness of services available at home and understand the roles and accountabilities of other agencies and professionals in order to refer, liaise, consult, and work effectively in order to meet identified needs.
