Summary
MAIN DUTIES AND RESPONSIBILITIES Royal Free World Class Values The post holder will offer World Class Care to service users, staff, colleagues, clients and patients alike. CLINICAL RESPONSIBILITIES JOB SUMMARY: To contribute to the clinical services of a CAMHS neurodevelopmental team as required, with particular focus on triage and screening of new patient referrals. Working alongside the team referrals coordinator to support with all aspects of their role, including receiving and processing referrals, independently chasing outstanding information and screening information. Attending, together with the referrals coordinator, weekly meetings with partner agencies to discuss new referrals and follow up on any pending referrals To collect additional information necessary to assess the appropriateness of referrals to our team, through contacting families, young people and referrers To consult with referrals coordinator about decisions regarding urgency and appropriateness of referrals Implementing triage decisions in conjunction with referrals coordinator Liaising with healthcare professionals such as GPs and community CAMHS teams to discuss referrals to the team and informing referrers of the outcome of their referrals. Being a point of contact with services, referrers and with families for newly referred young people. Introducing the service according to the Royal Free values (positively welcoming, actively respectful, clearly communicating and visibly reassuring). Liaising with other professionals such as GPs, social workers, schools, CAMHS clinicians and other healthcare professionals To keep accurate records of all information relating to referrals to the team in conjunction with administrative team Liaising with families in relation to their child's referral; they may have limited knowledge of the referral and assessment process and may need information accordingly. Parents may sometimes be in a highly emotional or distressed state of mind. Collecting clinical information to assess risk as required. If risk arises, discussing safety plans with parents/carers and signposting to resources prior to assessment. Contributing to outcome monitoring, auditing and service evaluation. Assisting in service research projects Other clinical and administrative duties at the needs of the service RESPONSIBILITY FOR PATIENTS To undertake such duties and responsibilities, appropriate to the grade of the post, as may be agreed with the clinical supervisor and line manager. RESPONSIBILITY FOR POLICY AND SERVICE DEVELOPMENT To contribute to service development as required, with support from senior team members RESPONSIBILITY FOR FINANCIAL AND PHYSICAL RESOURCES To use available resources responsibly and to be accountable for their use to postholders professional and service manager RESPONSIBILITY FOR LEADING AND MANAGING n/a RESPONSIBILITY FOR INFORMATION RESOURCES To participate fully and implement standards devised from Trusts clinical and research governance activity. To maintain the highest standards of clinical record keeping including electronic data entry and recording, report writing and the responsible exercise of professional self-governance in accordance with professional codes of practice of the Health Professions Council and Trust data protection policies and procedures. RESPONSIBILITY FOR RESEARCH AND DEVELOPMENT To contribute to ongoing research and development activities of the department through the timely provision of information and analysis in consultation with the postholders professional and service manager. GENERAL RESPONSIBILITIES Infection Control Infection control is everyones responsibility. All staff, both clinical and non clinical, are required to adhere to the Trusts Infection Prevention and Control policies and procedures and the Health Act (2006) Code of Practice for the prevention and control healthcare associated infections and make every effort to maintain high standards of infection control at all times thereby reducing the risk of Healthcare Associated infections. It is the duty of every member of staff to take personal responsibility for the prevention and control of infection, as laid down in the Trusts polices and procedures which reflect the statutory requirements of the Hygiene Code. To work in close collaboration with the Infection Control Team. To ensure that monitoring of clinical practice is undertaken at the agreed frequency. To ensure that the ward environments are cleaned and maintained to the highest standards; ensuring that shortfalls are rectified, or escalate as necessary. To ensure that all relevant monitoring data and issues are provided to the Directorates Governance structures. To ensure that all staff are released to attend infection control-related educational sessions and staff with specialist roles, e.g. link practitioners, are released to undertake their duties. Co-operate with the employer in ensuring that all statutory and other requirements are complied with. Confidentiality & Data Protection The post holder has a responsibility to comply with the Data Protection Act 1998 and maintain confidentiality of staff, patients and Trust business.
