Summary
Applications are invited for a 2-year 0.5 wte secondment to the role of Project Lead for Transition to undertake work to consider the issues and potential solutions for transition in Wales and lead the forthcoming Transition Implementation Network. This will report to the Child Health Network. It will take forward the recommendations from Welsh Governments review of NHS Wales implementation of the national Transition and Handover Guidance published in February 2022. The implementation network will therefore work closely with NHS Wales, including transition leads and the Strategic Clinical Networks and National Programmes in NHS WE.
