Summary


We are looking for a pro-active, highly motivated individual to join the Bracknell Intermediate Care Rehabilitation Unit MDT in the role of a Discharge Co Ordinator. You will be able to work both autonomously and as part of the discharge team. The applicant will be expected to be enthusiastic, organised with excellent communication and IT skills.

This will be 4-month secondment; further information about the role can be found within the attached job description and person specification. This is a great opportunity and would be suitable for a current band 3 or band 4 looking to develop their skills, experience, relationship with community teams and Adult Social Care. Appropriate training and support will be given.

This is a full time Monday – Friday role, early shift. Part time and job share will be considered.

An exciting opportunity has arisen for an experienced individual to apply to join the discharge team based at Bracknell Intermediate Care Rehabilitation Unit.

You will be supporting discharge planning for patients with complex health and social care needs. You will be required to work with members of the multidisciplinary team to guide patients andfamilies through discharge pathways.

You will have excellent communication skills with a flair for problem solving and the ability to work well under pressure. You will need to have the ability to work flexibly and independently but also as part of a wide multidisciplinary team.

As part of an MDT at rehabilitation unit at Bracknell it is expected that you work in partnership with social services and external stake holders, patients, and their carers to proactively support and facilitate timely and safe discharge from hospital to home or onward care settings.

To provide a single point of contact on a named ward for patients, families, carers, and associated people and co-ordinate/contribute to the safe and timely hospital discharge in partnership with other multi-disciplinary colleagues.



Maintain momentum of discharge planning throughout the entire process; supporting and working in partnership with other members of the MDT; doctors, nurses, occupational therapists, physiotherapists and other Hospital and Discharge Team members and constructively challenging where appropriate decisions with regards to discharge planning.

To screen patients on admission and identify those who will require further social care assessment and input and contact relevant.
