Summary
Any reasonable duties to be undertaken to achieve the following: Delivery of Pulmonary Rehabilitation groups face to face and virtual, contacting patients, and assisting with the service administration for the Pulmonary Rehabilitation Service across South and South East Staffordshire Work will be carried out in community settings e.g. village halls/equivalent or virtually. Undertake highly skilled support work with people who have, respiratory disorders including post covid syndrome if patients meet the service criteria The post holder will (in allocated time) act as a point of contact for all communication for the team. Principal Responsibilities Working to agreed competencies and protocols and guided by the registered practitioners, the post holder will implement a pulmonary rehabilitation plan i.e. carry out planned intervention as prescribed by registered practitioner or make minor adjustments to this following agreed protocols and guidelines liaising with the registered practitioner as necessary (as per service competencies) to patients and also their carer/relatives where relevant The post holder will be working under indirect supervision of the registered practitioners in the team. This will involve taking responsibility for an identified and delegated workload following screening and /or assessment by a registered practitioner. Leading and assisting with group exercise and advice sessions. Monitoring progress and feeding back to the registered practitioner. Be involved in the assessment of clinical outcome measures on both pre and post programme assessments and report on Minimal Clinical Improvement Difference (MCID) in post programme communication to GP Facilitate patients in completing their Patient Reported Outcome Measures (PROMs) both at pre-and post assessment Capture patient evaluations and feedback Audit and evaluate programme delivery and effectiveness To use motivational skills to encourage patients to self-manage their conditions. Provide advice and information to individuals on how to manage their own condition i.e. support patients with self care and self management of their condition. This may be carried out individually or in groups. This includes raising awareness of health and wellbeing. Deliver exercise sessions to improve health and wellbeing To utilise the Spirit clinical information system to retrieve patient information, register referrals and discharges, deal with tasks, type and send letters Prepare responses/letters/acknowledgments and requests for the Pulmonary Rehab clinical lead To ensure written documentation is accurate and disseminated to other professionals within the team To be a first point of contact for service users, answering the telephone and dealing with enquiries in a sensitive manner to document and convey messages to the team To arrange appointments for assessments consultations face to face and/or virtual. To process audit statistics for relevant personnel and assist with inputting audit data onto the ongoing national audit for pulmonary rehabilitation. To treat all patients as individuals, respecting their privacy and dignity at all times. Promote effective communication with, for and about individuals i.e. patients, other team members and other services/agencies in a manner appropriate to the setting and the individuals needs. This may involve adapting methods to overcome barriers to understanding. Communication may be verbal, written or electronic. Communicate with, and complete records for individuals i.e. maintain contemporaneous medical records (written & electronic) in line with Spirit policies. Relate to & interact with individuals and groups using encouragement, sensitivity, support and interventions as necessary. As classes are run the candidate must be comfortable with communicating to a range of individuals within a group environment both face to face and virtually. Maintain effective communication with your team and others you come into contact with. Comply with legal requirements for patient/client confidentiality. To have an awareness of current national standards and guidelines relevant to Pulmonary Rehabilitation, Respiratory Disease and improve practice and health outcomes where appropriate. Participate in CPD (continuing professional development) opportunities to keep up to date with evidence-based knowledge and competence in all aspects of your role Recognise and work within your own competence Partake in clinical supervision for your own development and maintain a record of your own personal development. Promote effective communication and relationships within the team. Prioritise, organise and manage your own workload in a manner that maintains and promotes quality. Support and participate in shared learning across the team. Undergo all mandatory training in line with role requirements. Understand how to gain valid, informed consent and to be able to work within a legal framework with patients who lack capacity to consent to treatment. Participate in the assessment and management of clinical risk. Participate in investigation of incidents and events as required. Specific Skills/Competencies/Qualifications/Person Specification GCSE level English and Maths or equivalent NVQ L3 (or equivalent) in appropriate field or working towards Willing and able to participate in competency based training programme Previous experience working and communicating with the public in a social care/health care setting Experience of working in a team Experience of working in the community setting Experience of teaching and instructing individuals and groups Experience of dealing with adult clients Experience of working with cardio respiratory patients IT literate Has an understanding of rehabilitation and reablement Has an understanding of confidentiality and Data protection Has an interest in respiratory care Able to work on own following agreed protocols, seeking help appropriately particularly around unforeseen circumstances Demonstrates positive attitudes and behaviours Good communication skills Car driver Ability to work in locations across South and South East Staffordshire
