Summary

The role is primarily to work within our SDS (Same Day Service) Team in providing a same day urgent treatment service and telephone triage. 1.Clinical 1.1. To provide a holistic, autonomous, and clinical nursing service by attending to referrals and self-referrals of practice patients. Making an assessment of their physical, psychological and social needs, implementing an agreed management plan with patients including appropriate prescribing and referral. 2. Independent/Supplementary Prescribing 2.1. Provide information and advice on prescribed or over-the-counter medications, side effects and interactions. 2.2. Manage medicines including prescribing and administering medication and appliances for patients, as an Independent/Supplementary Prescriber, or within Patient Group Directions. 2.3. To take responsibility, as a designated independent nurse prescriber, to prescribe medicines and appliances/dressings from the relevant national formulary for individual patients receiving care for a condition for which the nurse has clinical responsibility. 2.4. To maintain competency to practice safely as an independent/supplementary prescriber. 2.5. To comply with all legal and NMC requirements of prescribing practice. 3. Research 3.1. To ensure Clinical Nursing activities are evidence based. 3.2. When requested, to undertake an active role in carrying out research studies, independently or in conjunction with other health professionals. 4.Management 4.1. To ensure Nursing activities remain within broad policy, objectives of the organisation and a major role in developing policy objectives. 4.2. To report, document, and evaluate the work of the Nurse Practitioner and to demonstrate its importance both to patients and the Practice. 4.Professional 4.1. To provide a high standard of Professional conduct and Nursing care at all times in accordance with the NMC. 4.2. To participate in teaching Nurse Learners, medical students and other students as appropriate who visit. 4.3. To attend relevant meetings, courses, seminars and keep professionally up-to-date. 4.4. Any other duties considered relevant to the Post.
