Summary


An exciting opportunity has arisen for Discharge Co-ordinators to join our Transfer of Care Hub working with colleagues from both health & social care.

We are seeking applicants with recent experience of providing direct health or social care to patients, hospital experience is desirable but not essential as full training will be provided.Evidence of customer care and excellent communication skills is an advantage for this role.

Educated to GCSE or equivalent level with evidence of good literacy and numeracy, as well as good communication skills..

Persons appointed will be supported with competencies & development

You will be based at Eastbourne Hospital, however there may be a requirement on occasion for cross-site cover.

The role of the Discharge Co-Ordinator will be an integral part of the integrated discharge team and will be responsible for assisting the multi-agency teams with safe and timely discharges from the acute hospital setting. You will attend and feed information into enhanced discharge meetings, liaise with patients, carers and their families and where applicable attend patient panel meetings.
