We have an exciting opportunity for a passionate occupational therapist with highly specialist skills and knowledge in the field of frailty and geriatrics to join established Frailty Virtual Ward (Hospital at Home service). The role involves working closely with the Acute Frailty and Virtual Ward multidisciplinary team and is pivotal in ensuring all patients within the frailty Virtual ward receive the best possible care and service. The service focuses on patients who have been discharged from an inpatient wards or ED as well as those who are admitted directly from primary care or other community -based providers.

To provide specialist standard Occupational Therapy service to patients within the Frailty Virtual Ward which is delivered predominantly within the patient’s normal place of residence, facilitating and maximising patient potential within the service boundaries using a holistic approach.

To give highly specialist guidance, advice and education to other members of the Multidisciplinary Team (MDT) on matters relating to your specialist area.

Provide leadership for your team (in conjunction with the Service Manager) in planning, coordinating, delivering and evaluating the Occupational Therapy service provided to patients within your area.

Patients across north east London are benefitting from two new state of the art theatres at KGH and our Community Diagnostic Centres at Barking Community Hospital and St George’s Health and Wellbeing Hub will significantly increase the number of scans that can be carried out.

We’re looking forward to introducing an electronic patient record next year. This will mean the records of any patient visiting one of the seven hospitals run by BHRUT and Barts Health will be accessible to the clinical teams. It’ll make things easier for staff and will be better for patients.
