Crynodeb
To ensure that patients are referred onto other health care professionals and services to ensure a seamless, high-quality pathway of care for them, their families and carers. To work in conjunction with the Urology CNS team based at Westmorland Street (UCLH), MDT and other CNS colleagues to establish and set measurable standards of care and ensure the delivery of a high-quality service for patients, using nursing audit and other measurement tools. There will be an expectation to attend UCLH where needed (at most one day per week). To establish and maintain close working relationships with the Hospital and relevant Community Palliative care teams. To ensure accurate and comprehensive documentation in all patients' notes which include the trust Electronic Patient Records and Somerset Cancer database system. Develop knowledge base and clinical expertise in expected toxicities and late effects of urological cancer treatment. To work as part of the MDTs core member and actively participate in the National Cancer Peer review process. To regularly attend the Cancer Clinical Nurse Specialist Forum and participate in the agreed projects/work streams as agreed by the group. To provide CNS cover for cancer/Urology CNS colleagues during leave and absence within scope of practice To be competent in managing your own timetable and caseload. This includes managing the nurse led hormone therapy clinic with minimal supervision from Urology consultant. Attend and speak at network, regional and national meetings. To support the urgent (2 Week Wait) referral process, for patients who have a suspected diagnosis of prostate cancer. Ability to manage own time effectively.
