Summary
Role specific objectives include: Stakeholder Engagement to scope a place-based non-clinical model for Norfolk and Waveney: To facilitate co-design of the model with stakeholders to include people affected by cancer, carers and communities. To map existing community support and information services in Norfolk and Waveney To review processes for sharing personalised care interventions between primary and secondary care, with the aim to identify and implement any improvements. Some example personalised care interventions are Holistic Needs Assessments (HNAs), Personalised Care and Support Plans and End of Treatment Summaries and Cancer Care Reviews. Improving how the information is shared will help to avoid duplication, ensure that any issues are followed up between primary and secondary care and so that the patient may only have to tell their story once if appropriate. To review the quality of how Cancer Care Reviews are delivered by primary care and to make recommendations for their improvement. Integrate and awareness of the Core20PLUS5 agenda into the approach. Facilitate stakeholder engagement events to raise awareness of the ICJ model, personalised care interventions, Macmillan templates and cancer training to support the delivery of personalised care in cancer. To scope alternative models if required. To set up and manage a Stakeholder Steering Group to oversee the project and keep stakeholders informed on progress. To provide monthly progress reports To undertake an evaluation and report on findings.
