Summary
Undertake clinical nursing practice at an advanced level using knowledge and clinical skills to deliver holistic care to people accessing primary healthcare services. Triage, assess, diagnose and treat patients attending primary healthcare services with a range of acute, non-acute and chronic medical conditions. Directly admit patients to secondary care in acute medical need. Refer patients as appropriate to other members of the MDT, secondary care and other statutory and voluntary organisations. Referral to x-ray after having undertaken x-ray training as required by the designated hospital trust. Undertake and interpret a range of diagnostics tests where appropriate. Make clinical judgements appropriate to the expectations and demands of the role. Work and adhere to evidence based practice, recognised clinical guidelines and practice policies. Advise, support and mentor other members of the clinical team. Independent Non-Medical Prescriber. To prescribe safe, effective and appropriate medication as defined by current guidance. Effectively manage own time, workload and resources. Undertake home visits for acutely unwell housebound patients if required Identify safeguarding concerns and make referrals as appropriate Create, implement and manage IPC policies on behalf of the Practice
