Summary
A. Working Relationships and Communications Requirements of the Job. 1. To communicate complex patient related information effectively ensuring collaborativeworking with multidisciplinary team members across health, education, social servicesand the voluntary sector. Dependent on the rotation to work in particular with teachingstaff, nursery staff or medical/ nursing staff to ensure the delivery of a well-co-ordinatedcare/individual education plans/programmes, facilitating the development of physicalskills. 2. To work collaboratively with the multidisciplinary team within hospital, Alexandra AvenueHealth and Social care centre, mainstream schools, special schools or in nurserysettings, dependent on the rotation. 3. To share information with others, observing confidentiality and data protection guidelinesand having due regard for local guidance on sharing information with colleagues in health,education, social services and the voluntary sector.
