Summary
· Carrying out initial risk and care needs assessments in respect of individual service users in the service user’s home and put together a person centred care plan.
· Work in partnership with service users, their family, carers, advocates, healthcare professionals and others to ensure the seamless delivery of holistic packages of individual care support
· Work with care coordinators to support the planning of effective two weekly care worker rotas, with due regard for the personalised needs of the service user and the particular skills and attributes of the carer
· Carry out regular reviews of service user care
· Promote a safe working environment and safe systems of work in respect of cares in the field and those that might be affected by their activities
· Understand and follow hazard and accident reporting procedures
· Be observant to any changes in service users’ condition and circumstances and act appropriately, when these change.

It is essential to the role that you are a driver and have access to your own vehicle. We are looking for someone who is experienced in the job role and has good knowledge of CQC regulations. It is also important that you are able to navigate only systems.
This is a full time role working Monday to Friday 9am - 5pm. However, due to the nature of this job a degree of flexibility will be required.
