Summary
Alcedo Care Carlisle, part of the Alcedo Care Group, which is one of the fastest expanding health care providers in the North West Region is looking for an experienced Care Assessor to join its professional and dynamic Management Team.
The role will be permanently based in our Carlisle office in West Cumbria.
We are looking for someone who is experienced in carrying out assessments of potential new clients in their own homes or other care settings.
You will be responsible for meeting with the clients, their relatives and possibly other healthcare professionals. You will fully assess their requirements and the support they require and develop a person-centred, comprehensive care plan.
We use the OneTouch electronic monitoring system which is fully integrated with all aspects of our client’s care plan and it will be your responsibility to ensure this is kept up to date, to an exceptionally high standard and ensure the system is at 100% compliance at all times.
Responsibilities include but are not limited to;
• New client assessments
• Maintaining care plans; including care management on OneTouch
• Participate in the on-call out-of-hours service, on a rotational basis alongside other members of the management team.
• Resolving alerts/Reducing alerts on the OneTouch system
• Care plan reviews and improving the quality of existing care plans
• Actioning audits carried out by the manager each month
• Complete weekly reports for the Registered Manager to be made aware of and sign off
• Support Care staff regarding their duties outlined in the care plan
• Liaise with our Care Coordinator to ensure staff/client compatibility
We are looking for someone who:
• Has an NVQ Level 3 in Health & Social Care
• Holds a full UK driving licence and has access to their own car.
• Is experienced in undertaking care assessments
• Previous Healthcare experience
• Is compassionate and caring.
• Will always represent Alcedo Care in a professional and dedicated manner
• Can communicate well
• Is computer literate
• Is hard-working and flexible
