Summary

Key results from the job holder Work as an autonomous clinician in a defined area to develop and provide expert clinical advice, support and care to a defined group of patients. Develop and use advanced clinical skills to analyse and interpret history of illness, presenting symptoms and physical findings to enable diagnosis, planning and treatment of patients. Empower patients and their families through appropriate support and education to manage their long term illness or disability. Develop patient self-management plans. Facilitate the integration of patients goals for health and social care plans and future health needs. Develop working relationships with primary and secondary care clinicians to ensure patients care is of a high quality and appropriately delivered. Establish partnerships and networks with social services, voluntary sector and other agencies to ensure patients access all resources available, working closely with social care teams. Provide and co-ordinate the interface between acute, primary, community and social care settings in order to reduce unnecessary attendances/admissions to hospital and facilitate early discharge if possible Be part of an integrated community team attending regular team meetings to plan and organise patient management plans. Collect, collate, evaluate and report clinical information, maintain accurate patient records related to assessment and care planning. Develop clinical protocols and guidelines within own area of practice to ensure quality of care at all times. Review medication, discuss side effects and where appropriate prescribe relevant medication or liaise with medical staff. Regularly evaluate practice to ensure continuous improvement. Lead on professional quality and safety for vulnerable people with highly complex needs, co-morbidity and long term conditions. Act as a resource to all staff in delivery of clinical care to patients. Act as an advocate for the patient negotiating and consulting with other clinicians and associated staff to ensure high quality care. To provide a visible and accessible point of contact for patients and carers, ensuring high standards of individual patient care. Work with other clinicians in examining episodes of care delivery, critical incidents and individual care plans to improve and develop services. Work across professional boundaries providing expert clinical knowledge whilst developing new skills. Identify own learning needs and subsequently plan, implement and evaluate strategies to meet identified needs. Ensure own compliance with mandatory training programmes and own ongoing professional development and development of the advance practitioner role. Engage in clinical supervision and self-reflection to develop own clinical standards and work practices. Planning and Organisational Duties Contribute to professional and policy making activities that impact on development of clinical practice. Communications and Key Working Relationships Provide advanced clinical knowledge within own scope of practice to clinical colleagues. Take a lead role in evaluation of clinical practice to identify and implement change to continuously improve the standards of clinical care. Provide leadership and co-ordination across professional boundaries working closely with other clinicians within the same area of practice. Act as a key role model within the locality teams, offering advice to multi-disciplinary team members on case management Responsibility for Finance Support Manager in achieving budget requirements Responsibility for Human Resources Participate in the recruitment and selection of staff as required. Participate in the induction and orientation programmes for newly appointed staff including disciplines other than nursing. In the context of service development, contribute to development and planning of an appropriate workforce as required. Ensure prompt recording and reporting of accident/incidents Report sickness and absence in accordance with Trust policy. To ensure compliance of self and others with Health & Safety Policies and provide a level knowledge, training and support which contributes to providing a safe working environment, meeting the requirements of the Health & Safety at Work Act. Responsibility for Health & Safety Compliance with the Health & Safety at Work Act 1974 the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions. Responsibility for Teaching Participate in education, training and mentorship of other clinicians. Participate in pre and post registration training for staff from multi professional backgrounds.
