Summary


We now have a vacancy for a Band 4, IBD MDT Co Ordinator. This is part time, 26.5 hours per week and based at Fairfield General Hospital.

You will use specialist knowledge of the patient pathway to practively influence the tracking records using high levels of communication to expedite appointments/procedures as necessary and to resolve issues which could lead to target failures. You will persistently track, update and upload all the data referring to the MDT referrals and update electronic tracking records, specialist or local MDT teams and local services team accordingly and provide full assistance to the clinical team in the co-ordination of the Multi-Disciplinary Team Meetings and represent the MDT Teams to disseminate the information regarding the patient pathway.

You will communicate effectively with all service teams, senior clinicians and other Trust senior managers and provide support to the MDT meetings, including recording actions and outcomes and national data collection, and other data collection identified by the MDT.
• Completing patient information with support available from the clinical team and clinical nurse specialist.
• To communicate with referrers regarding incomplete referrals/additional information required prior to MDT discussion. Support available from the clinical team and nurse specialist.
• Ensuring all relevant information and results are available for the MDT meeting to allow patient treatment planning.
• To ensure an online joining link for the meeting is made available to all attendees, or in the case of a face to face meeting, a room is available, and all technical equipment is functional and set up prior to the meeting.
• To circulate the MDT list to relevant parties ahead of the meeting.
• To document the outcomes of management plans, and disseminate to the members of the multidisciplinary team meeting and referrers, some of which could contain emotional/distressing infromation.
• To understand the management plans and initiate the implementation of recommendations as required. This may include chasing appointments etc. To discuss with the clinical team any aspect of the patient pathway that the co-ordinator is uncertain of.
• To ensure all patients are discussed at the MDT at appropriate points of the patient pathway.
• Tracking to be updated daily.
