Summary
There are 67 beds across these two community hospital wards for geriatric and post-acute rehabilitation. The case mix is broad and includes those following orthopaedic surgery, trauma and amputation as well as acute physical illness in older people. The majority of patients are admitted from Derriford Hospital following an acute episode of care. There is an experienced multidisciplinary team across these wards including nursing, physiotherapy, occupational therapy, pharmacist, social worker, dietician and psychiatric liaison nurse support. Following a successful pilot we have up to 8 End Of Life beds in side rooms across the two wards which are supported by the palliative care team. Out of hours cover is provided through the senior nurse on site, supported by off-site hospital wide on-call junior doctor and senior on-call rota (in partnership with Livewell Southwest [LSW]). There are 6 nursing and medical teams across the two wards. Each patient is seen weekly by a senior doctor, and there is a weekly MDT to discuss all patients. You will be responsible for the medical care for patients across the two wards. You will cross cover absent colleagues too.
