Inclusion & Transition Support Worker required, 3 days per week (Mon to Wed 8.30 – 3.30) based at The Phoenix Learning Centre in Hinckley.Full time could be available, although the other 2 days (Thurs/Fri) would be based in Leicester.
We are seeking to appoint a committed, caring, flexible and resilient professional to complement the work of our exceptional team. This role is vital in helping students successfully reintegrate into their home school, ensuring they feel confident and supported during this critical period.
In September 2024, we opened a new provision based in Hinckley – The Phoenix Learning Centre. This is a regional hub of the Children’s Hospital School offering short-term (12 week) placements for secondary aged students currently unable to attend school for health reasons. The provision supports up to 12 students with an academic and wellbeing curriculum, alongside the development of a transition plan to support a successful reintegration to their school.
Applicants should have experience of working with young people, and a belief in a truly personalised provision for each child. There will be times during the week when you will support teaching staff in curriculum activities, although the main purpose of this role is to develop and implement personalised transition plans with pupils, families and the home school. This will involve supporting pupils with school visits as appropriate.
