Summary
Job Description
The NE Social Care Worker (Carers) will work within the Glasgow Carer's Partnership Model to identify unpaid carers, formally assess and implement outcome-based support plans. The successful candidate will require a good understanding of the needs of unpaid carers and have excellent communication and engagement skills. The Social Care Worker (Carers) will be managed within and as part of the Glasgow North East Carers Team and will be required to work in partnership with a range of partnership organisations. Applicants must have the ability to demonstrate initiative, develop and implement ideas and work efficiently. The post will be primarily located in the North East however the successful applicant will be required to facilitate Carer team duty remit at the HSC Connect building on a very regular basis. Knowledge and understanding of the Carers (Scotland) Act 2016 would be beneficial. Membership of the Protection of Vulnerable Groups Scheme will be essential on taking up appointment. Please note successful applicant for the above position will be required to pay for their PVG Membership/or Scheme Checks. These posts are excepted under Rehabilitation of Offenders Act 1974 (Exclusions and Exceptions) (Scotland) Order 2003 (as amended).
