Summary


We are seeking to recruit a dynamic Band 3 Community Healthcare Practitioner (Part Time) on a fixed term contract for 6 months based at the Heart of Hounslow. The successful candidate may be required to cover other EIP location and visit service user’s home as per service need. This role could be extended subject to funding.

The individual must be enthusiastic, committed, compassionate and have experienced working with service users experiencing psychosis. If you don’t have this experience, support will be provided.

The candidate applying for this role should not require visa sponsorship now or in the future.

The post holder willbe responsible for contributing to healthcare under the supervision of a registered member of staff providing a recovery focused approach to physical, psychological, social, recreational and spiritual care to service users.

The post holder will carry out duties as designated by the Team Leader and report on the patient’s condition. They will maintain the orderliness, hygiene and safety of the unit/ community environment and will be required to undertake routine clerical duties.

The post holder will be responsible for the EIP physical health needs of service users provision as part of the 7 core intervention – this will include taking blood pressure, blood glucose, height / weight, cholesterol checks, general physical checks as required. This will also entail liaison with GP’s around blood test results.

Physical health checks are one of the core interventions for clients under and EIP team. To carry out these checks for young people and working age adults.
