Summary
The post holder will be responsible for assessment of care needs, development, implementation and evaluation of programmes of care for patients within the Team Leader’s sphere of care.
The post holder works without direct supervision and is responsible for nursing care for the geographical area including weekends and evenings.
The community team of West Essex is part of the wider Hertfordshire and West Essex Integrated Care System supporting Out of Hospital Care to support West Essex Residents. The post holder will be working as part of the Integrated Neighbourhood Team supporting the Uttlesford North Primary Care Network with a focus on prevention of admissions and facilitating hospital discharges
1. Takes responsibility for a patient or a defined group of patients within their own homes or elsewhere as defined by the Senior Nurse.
2. Undertake comprehensive nursing assessments to identify the patient’s physical and environmental needs.
3. Plans, evaluates and prioritises interventions and care for patients within his/her sphere of responsibility.
4. Leads the clinical nursing management of the caseload’s most dependent patients. Determines, in liaison with the Senior Nurse the frequency, type and monitors the efficacy of patient care offered.
1. Regularly review caseload patient care, informing the Senior Nurse of any changes made.
2. Establish priorities in his/her workload – organising his/her workload to make effective use of time.
3. To act up in the Senior Nurse’s absence.
4. Responsible for the delivery of nursing care for a geographical area including weekends and evenings.

Clinical & Professional Duties

Patient/Client Care
1. The post holder will be directly involved with a variety of patients on a day to day
basis offering direct and indirect care, i.e. personal care and advice by other means.
2. Participate in evidence-based practice.
Communication Skills
1. Liaise with other agencies and members of Primary Health Care Team and secondary care.
2. Actively promote, develop and support working with the GP Practice and take part in joint initiatives to help meet health targets.
3. Provide clinical leadership to newly qualified/newly appointed staff and Health Care Assistant team colleagues.
4. Provide area Staff Nurse and Health Care Assistant supervision as and when required.
5. Provide professional support (formal and informal) to staff, carers and family members in residential homes as required.
6. Annual leave and study leave to be negotiated with the Team Leader.
Education and Research
1. To supervise the delivery of care by other staff, including students, in order to meet the identified patient needs, according to staff competencies.
2. To contribute and participate in orientation programmes for new staff.
3. Actively promote health issues at appropriate opportunities and participate in Health Promotion campaigns sponsored by the Trust.
4. Participate in the training of students and undertake appraisal when required.
5. To participate in further education as appropriate and can be responsible for own professional development.
6. To proactively participate in the team’s health and safety management of team members, patients and general public.
7. To participate in clinical supervision sessions on a regular basis.
8. Demonstrate knowledge of an active use of research in nursing intervention.
9. Require to teach, demonstrate to Carers, Multi-disciplinary Team, patients and/or their families as necessary.
Managerial Responsibilities
1. Deputise for the District Nursing Sister where appropriate, ensuring that the provision and supervision of nursing care is met.
2. With the support of the District Nursing Sister or Scheduled Care Manager provide information/investigate complaints as required and assist in taking appropriate action to reduce further occurrence.
3. Ensure equipment is regularly maintained.

2. Accurately and systematically record information
3. Input data and information accurately.
4. Ensure the necessary precautions are adhered when transferring information, only disclosing patient identifiable information in line with Caldicott and the Data Protection Act
