Assessment and Review Officer – Home First
Home First is an Urgent Care and Reablement Service Provided by Hoople, As a team, we enable those living in Herefordshire to achieve independence and live more fulfilling lives!
Reablement focuses on promoting independence by supporting people to do as much as possible for themselves both physically and psychologically. It aims to do this by supporting people to continue living at home, preventing admissions into hospital or residential settings, and facilitating timely discharges.
Reablement is a short-term service working with individuals for up to 6 weeks.
As an Assessment and Review Officer working for Home First, You will be the first to visit an individual in their own home and together you will complete an initial assessment, risk assessments, and identify the individual’s daily living tasks that can be re-abled.
You will create a support plan which focuses on goal planning for the Reablement Workers to follow alongside the individual to achieve their outcomes which are reviewed weekly during the service, and you will also assess and prescribe items of equipment to enable independence and reduce the need for ongoing support.
You will also be responsible for writing Medication Administration records and Identifying concerns that affects an individual’s physical and psychological wellbeing.
We believe that by supporting customers to identify and achieve their personal goals and outcomes, and by embedding personalisation, individuals will have choice and control over how their support will be delivered, this will have a positive impact on health and wellbeing.
The role requires working a variation of hours between 7.00am and 22:00pm, along with working 2 weekends in a one month period, the service operates a set 4 week rolling rota pattern and individual working pattern to be confirmed at interview,
Key responsibilities;
- On a daily basis you will liaise with colleagues, individuals, health professionals and the public, either face to face or over the telephone, so it is important you have a courteous, professional manner.
- You must be able to work as part of a team but also have the ability to work on your own initiative.
- You must be able to drive and support the service both North and South of the county.

Paid mileage – 45p Per Mile

Funded Qualifications
Experience and Qualifications
A level 2 or 3 qualification in Health and Social Care is essential or demonstrable experience within a previous role.
A minimum of 1 years, experience in Health and/ or Social Care is essential.
