You may be required to work in other designated locations of theTrustaswellasyourprimary base.Inparticular,flexibilityisrequired across the three main hospital sites (Leicester RoyalInfirmary, Leicester General Hospital and Glenfield Hospital). Ifyourinitiallocationisoneofthesesitesexcesstravelreimbursementwouldnotapplyforapermanentortemporary change of base.

Accountable to:DischargeMatron.Deputy Head of Nursing/ Head of Nursing (professionally accountabletolead AHPtherapist)
To be based in the UHL but working collaboratively across CMG’s and with community partners to identify and proactively work with those individuals who are High Frequency Users of the Emergency Department . To promote and advocate for the care provision and the smooth transition of patients in the acute care areas (Emergency Department & Emergency Floor) and ensure that patients are cared for in the appropriate environment.The practitioner will act as the clinical and professional resource to the multi- professional, multi-agency team, patients and carers identified as a High Frequency User of the Emergency Department.
To be based in the UHL but working within the Primary and Secondary interface to identify and proactively work with the multidisciplinary team. To promote the care provision and the smooth transition of patients in the acute care areas (Emergency Department & Emergency Floor) and ensure the patient is cared for in the appropriate environment.
The practitioner will act as the clinical and professional resource to the multi- professional team, patients and carers within their specialist service.

• Work autonomously, as part of the University Hospitals Of Leicester High frequency Usersprogram and the Leicester Royal Infirmary Emergency Floor,whilst building and maintainingrelationshipsandnetworkswithPartnersbothinternaltoUHLandexternalwithintheCommunity.
• To work alongside the Emergency Department MDT and High frequency users MDT toidentify and respond to unmet needs in those individuals who present as High FrequencyUsers to the Emergency Department. To advocate on behalf of patients to promote safe andtimelydischargefromtheEmergencyDepartmentandtoavoidadmissioninto hospital
• Ensure that robust systems are in place for effective monitoring of performance againstnational and local indicators and any other reporting requirements. The post holder will beexpectedtobefamiliar with alllocaland NIHCEguidance relevanttothespeciality.
• Todevelopandmaintainanaccurateandeffectiverecordkeepingsystem,collectingstatistical data and writing reports on a regular basis, and presenting to appropriate forumswithinand externaltoTrust.
