Summary
The Senior Patient flow coordinator for Housing will improve the quality of care for homeless patients within the Hospital through:·specialist advice·advocacy·support and quality discharge interventions, with particular focus on accommodation options.
The Senior Patient flow coordinator for housing will work with the housing team as part of the multi-disciplinary team. The aim will be to provide enhanced inpatient care and ensure safe, appropriate, and sustainable hospital discharges that meet the needs of individual homeless patients.
This role is aimed to reduce delayed and premature discharges, and reduce unscheduled admissions, repeat admissions and A&E attendances. As a key part of a multi-disciplinary service, the role aims to help patients improve and maintain their health by linking them with suitable housing support and other relevant services. The patient flow coordinator will support them to access accommodation, benefits, primary health care and to attend follow up health and other appointments following discharge.
The post holder will ensure safe, appropriate, effective, and sustainable discharges that meet the needs of the homeless patient. The post holder will work in partnership with members of their own multi-disciplinary team as well as other hospital teams and outside agencies to achieve successful discharges.
Patient Case Management
· To deliver individualised advice to patients liaising with the necessary support
· agencies/authorities to achieve the best possible outcome on discharge.
· To act in an advocacy role both within the clinical setting and with external agencies,
· using the expertise of peers in the multidisciplinary team as appropriate.
· To carry out holistic housing assessments that will help prove a patient’s homelessness,
· eligibility, and priority for housing support and to verify local connection.
· To accompany patients to Housing Options Services to provide advocacy and resolve
· disputes to achieve best results for patients.
· To immediately report any difficulties experienced in Housing Options
