The Role
We are seeking a dedicated Adult Social Care Worker to join our Social Care Community Team for People with Learning Disabilities (CTPLD). The team is responsible for meeting the social care needs of people with learning disabilities in North Somerset. This is an exciting and challenging time for Adult Care Services. We need an enthusiastic and motivated individual to join the team and assist in the development and reviewing of services being provided to people who use our service.

Key Responsibilities:
Assessment and Review care and support using the Care Act (2014)
Responsibility for care management of case load using a Strengths based approach.
Work alongside the duty social worker and provide flexible same day responses.
Build productive relationships with people who use our service, their carers and service providers, to ensure the best outcomes for individuals
Uphold North Somerset Council’s social care duties.
This position is exempt under the provisions of the Rehabilitation of Offenders Act (Exceptions) Order 1975 and is subject to an enhanced Disclosure and Barring Service check.

Valid driving licence and own transport, or be able to provide alternative, suitable method of travel is an essential condition for this role.

About You
We are looking for an individual who possesses the following skills and experience:

Essential Criteria:
Significant experience of working with people who have social care needs.
Ability to analyse information and recommend solutions to ensure best outcomes for the people who use our service.
Ability to write clear reports which are consistent and of a high quality
Ability to use a variety of IT systems competently.

Desirable Criteria:
Knowledge and experience of Safeguarding Adults
Knowledge and application of the Mental Capacity Act (2005)
