Summary

The Community Neuro Rehab Team is a multidisciplinary team working to provide specialist Neuro Physiotherapy, Occupational Therapy, Speech and Language Therapy, and Vestibular intervention to adults (18+) in patient's homes and appropriate community settings. We maximise independence and aim to provide patient centred, evidence based holistic care.
We’re looking for an Occupational Therapist with Neuro experience to join our multidisciplinary team.

We’re a very supportive team with Physiotherapists, Occupational Therapists, and Assistant Practitioners.
You will be responsible for responding to referrals along with other health professionals, completing a holistic assessment of need and associated risk assessments and completing initial follow up visits, as well as referring patients on to other services. You will also participate in the supervision of junior and support staff and at times assist senior staff and the service with a range of other tasks to maintain and improve a quality and evidence-based service.

The Occupational Therapy role will function as part of the multidisciplinary team in community care setting to provide a seamless service to patients. The post holder will be expected to effectively manage a caseload of patient’s co-morbidities as an autonomous practitioner, using evidence based patient centred principles to assess, plan, implement and evaluate interventions of the patient group. The post holder will provide leadership for junior staff, when appropriate, through supervision and appraisal.
The post holder will participate in planning, development and evaluation of services within a designated area/team, holding responsibility for defined projects.
The post holder takes a holistic approach in the assessment and treatment of patients, being responsible for the provision of appropriate, high-quality care to a defined caseload which may be of a complex nature, using the appropriate model of care and evidenced based practice. The post holder will work in partnership with other professionals to enable patients to be maintained in an appropriate environment. The post holder will participate as an active member of the multidisciplinary team in developing and delivering high quality and innovative services to patients. The post holder will promote effective teamwork, delegating activities appropriately, to the benefit of the patient and service delivery, and will contribute to the maintenance and development of the Occupational Therapy profession within the team and facilitate effective learning within the area of practice for all students and practitioners.
