Summary
See attached document Clinical To undertake assessment of individual patients & within your clinical practice the standards & policies of this speciality & evaluate the outcomes of treatments offered To carry out interventions which support the patients with their cardiac rehabilitation. This may include supporting the patient to maintain or develop new behaviours or practice behaviours which support them to self-manage their needs To perform functional capacity tests and create an exercise plan according to results To be responsible for demonstrating the exercise and the use of equipment and tools to patients
