Summary
To engage with individuals in the community and improve their lifestyle and self-management capacity by activating their assets: Apply behaviour change techniques to support complex individual lifestyle changes, e.g goals setting, confidence building Provide evidence based about a key public health agenda Social prescribe to other service that will enhance and sustain the outcome of the interventions. To manage own workload efficiently Contribute to and work as part of a team. To support existing services and develop new local approaches to meet the needs of client in the target group To undertake basic clinical interventions such as blood pressure Provide support to volunteers and other community partners where directed Participate in MDTs with partner organisations Undertake any other delegated work Any other function commensurate with competencies and job role
