ABOUT THE ROLE Your focus asTeam Leaderwill be tosupport thehome management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors,and staff. Other responsibilitieswillinclude: Leadingin the person-centred care planning processby implementing,monitoring,and undertaking monthly evaluations, completing care plan audits, developing action plans, andbeing responsible forcompletingthe action plans. Assistingin the smooth transition of any discharges from the home, including documentation, medication, andkeeping theresidents'next of kin fully informed, whereappropriate. Leadingwithmonitoringthewell-being of each resident, being aware of any changeimpactingcare and care planning andensuring that changing needs are met and written records are amended to reflect any variation. Practicingsafe systems of work across a range of tasks andmovingand handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff. ABOUT YOU Tosucceed in your application, you will live our values of caring, supportive, honest,respectful,and accountable in allyou do. Our ideal candidatemust: Have anNVQ 3 or equivalent in care for care homes. Twoyears experienceworking in a senior position with the residentgroup. Have the ability tocommunicate effectively both verbally and in writing. Demonstrate the ability to lead staff and act responsibly in the absence of themanager.
