Summary
An opportunity to deliver Group sessions to patients in the community with the flexibility of working from a number of bases or from Home. The classes are delivered in Norwich, Kelling and Wymondham. The pulmonary rehabilitation clinician will deliver comprehensive clinical triage and assessment of people referred to the pulmonary rehabilitation service to determine clinical baseline measures and assess clinical discharge outcomes. They also deliver the Pulmonary Rehabilitation programme (exercise, lifestyle, lung education) in a variety of settings, as part of an MDT, for people who have a diagnosed long term lung condition in collaboration with the wider multi-disciplinary community pulmonary rehabilitation team.
• To undertake comprehensive assessment of people referred to the pulmonary rehabilitation service to determine their baseline and discharge clinical outcome measures within a variety of settings.
• To plan and deliver an individual specialist pulmonary treatment programme using knowledge and evidence-based practice and treatment options. The treatment plan is to be based on effective intervention and patient agreed goals taking into account their psychosocial needs.
• To obtain the informed consent from patients for the interventions delivered by reviewing and reflecting with the patient their understanding of their problems and treatment options. To provide specialist advice and education regarding the service to other professionals within health, social care and the voluntary sector. This may involve liaison with other professionals such as GP’s and Consultants.
• To evaluate the progression of a patient’s problems and goal achievement with respect to the treatment plan and adjustment of the management plan as necessary.
• To assist with the triage of referrals to the pulmonary rehabilitation service effectively and efficiently to ensure the minimum of delay in meeting the patient's needs and to meet the agreed key performance indicators.
• To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.

Clinical
The post holder will:
• Assess, plan, implement and evaluate all aspects of patient care, and develop treatment/care plans that may need to be delivered from a range of options.
• Following a holistic assessment and in liaison with the multi-disciplinary team, devise an individualised care programme for each patient, modifying it as required.
• Assess patients’ holistic needs, communicating complex and sensitive information to patients and carers as to their assessment, diagnosis, and treatment plan.
• Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance.
• Be required to use tact and persuasive skills in order to ensure active participation in self- management programmes, in particular where there may be high levels of anxiety and/or depression.
• Demonstrate a high level of listening and communication skills and deal sensitively and confidentially with an individual's concerns, particularly at times of high levels of distress or anxiety.
• Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately.
• Be required to undertake risk assessments and act upon them appropriately.
• Ensure activity data and clinical information is recorded on SystmOne and completed on a daily basis.
• Demonstrate dexterity and co-ordination when using specialist equipment and fine tools, advanced sensory skills, manual and mobilising skills.
• Ensure informed consent is obtained prior to initiating interventions.
• May occasionally be exposed to bodily fluids or infected material and therefore must utilise universal precautions and adhere to infection control policies.
• Prepare clear and concise reports for GPs and other referrers to the service outlining the results of assessment and treatment outcomes.
Professional
The post holder will:
• Be responsible for ensuring workload for yourself is planned and prioritised according to service targets and needs.
• Ensure own paperwork and records are maintained, and those maintained by the team
• Be required to undertake risk assessments and act upon them appropriately.
• Be aware of budget and ensure all effort is made to work within this
• Actively contribute to multidisciplinary team meetings and other meetings where appropriate.
• Support all team members within the integrated team, respecting them, their roles and contributions.
• Attend in-house training as required to develop current job role
• Be able to make own travel arrangements to pulmonary rehabilitation programme venues, base and meetings etc. as required if unable to travel in the work vehicle provided.
• Be responsible for safe use and maintenance of equipment and supplies. Including the rental car.
• Remain accountable for own professional actions as determined by the professional bodies.
• Be required to demonstrate own duties to students, new starters and/or less experienced staff
• Provide mentorship for students undertaking pre/post registration course.
• Provide advice/training to less experienced staff and supervise and support other staff; and students where appropriate
• Mentor existing staff and provide training in specialist area as appropriate.
• Contribute to the induction process for new staff/students.
• Demonstrate basic IT/standard keyboard skills
• Demonstrate leadership skills within own competence.
• Maintain own continuous professional development.
• Be accountable for work delegated to others.
