Summary
We are looking for enthusiastic dedicated health-care workers to join our team, providing care at home in our local catchment area. In this role you will make a lasting difference to the lives of people through care that will have a positive impact on the patient, and the community for many years to come.
The Rapid Personalised Care Service (RPCS) is designed to offer care for a period of up to 12 weeks to enable patients with specialist palliative care needs to be cared for at home.

You will be delivering a high standard of compassionate care to patients in their own home, providing personal care and support to help to keep them at home, and allows us to reach more patients, helping them to achieve their preferred place of care, and death at home with the peace, dignity and serenity we know we can provide, giving family and friends support, and time to be together. As a palliative care provider we reduce pain symptoms and discomfort for patients, and support their welfare with an individually agreed plan, at a difficult time in their lives.
To work as a member of the team, delivering a high standard of compassionate personal care to patients in their own home including, supporting with washing, dressing, mouth care and medication administration.
For our staff we place great importance on practice development, clinical supervision and ongoing education. We are also committed to giving you more flexibility and control over your working life.
You will work covering rostered visits 7 days a week between 8am and 10pm.
We are passionate and committed to providing free care to over 2000 local people and their families. We have been inspected and rated ‘outstanding’ by the Care Quality Commission and you could be a part of this too.
