Summary


To work within the complex discharge team and the discharge lounge, developing skills within the role of complex discharge planning rotating between the discharge team and lounge on a 6-month basis.


• To provide care to a group of patients in accordance with the NMC Code of Professional Conduct,Trustpoliciesandprocedures.

· Ensureclinicalincidentsandrisksarereported.

· The post holder will be expected to work using evidence-based practice and research.

· Provisionsofthe Data Protection Act (1998) to maintain effective management of records ensuring information governance is always maintained.

· ComplywithTrustInfectionPreventionandControlpoliciestopreventharmto all.

· UseandupdatepatientinformationutilisingsystemssuchasVitalPacandVitalFloandPAS.
• To perform procedures such as, but not limited to, ECGs, IV administration, cannulation, Venepuncture, male and female catherterisation as well as wound management.

· To work within the complex discharge team and the discharge lounge to develop skills within the role of complex discharge planning rotating between the discharge team and lounge on a 6-month basis.

· To work closely with ward staff involved in discharge planning to ensure that the assessments and referrals for services relating to the patient’s discharge is completed comprehensively and in a timely manner.

· To support the established team in ensuring the delivery and maintenance of a high standard of carethatispatientcentredandevidencebased.

· To work with teams and specialties across the system to facilitate safe discharges in a timely manner.

· Adhere to the NMC Code of Professional Conduct, Guidelines and Standards and ensure colleagues do too.

· Provide high standards of evidence-based patient centred care and knowledge for all patients,respecting dignityandprivacy.

· Deliver high quality care for patients within the discharge process and ensure all discharge plans are robust toprovide aseamlessandcohesivetransitiontoothersettings.
• Attendmandatorytrainingas aminimum.
