Summary


To work as a member of the Rapid Response & Intermediate Care Team in (the community) in Aylesbury and surrounding villages providing nursing, social and rehabilitative care which may include some nursing, physiotherapy and occupational therapy-based interventions for designated patients (either individuals or a specific clinical group). The overall aims of the Rapid Response & Intermediate Care Team are to avoid inappropriate admission into the Acute Service and facilitate timely discharge into the community.

A full driving licence with business insurance and access to a car for work purposes.


• Providing general and specific care to adults within competencies
• Being an integral member of the Rapid Response & Intermediate Care (RRIC) team, providing defined delivery of care including nursing, social and therapy based rehabilitative care to patients in liaison with senior staff.
• Taking responsibility for designated elements of the patient treatment protocols and delivery of such treatments in patients’ own homes.
• Being responsible for the completion and potential delegation of non-clinical tasks within the team.
• Improve the care and support of patients in the community, so patients can remain safely in their own home for as long as possible
• Have an innovative approach, excellent communication skills and willingness to work with other members of the community teams to ensure high quality service delivery
• Willingness to further develop your clinical expertise in a range of areas.
