Qualifications
- Proven experience as a Support Worker - Minimum 6 months
- Level 3 in Health and Social Care - Desirable
- An empathetic and patient approach to supporting individuals with diverse needs
- Strong communication and interpersonal skills, with the ability to build rapport and trust
- Knowledge of person-centred care and a commitment to promoting individuals' independence and well-being
- Demonstrated ability to work effectively within a diverse and inclusive team environment
- A commitment to upholding ethical standards and respecting the rights and dignity of others
Responsibilities
- Provide person-centred support to individuals, taking into consideration their unique needs and preferences
- Assist in daily activities such as meal preparation, housekeeping, and personal care
- Support individuals in accessing community resources, social activities, and healthcare services
- Collaborate with healthcare professionals, social workers, and family members to ensure holistic care for individuals
- Maintain accurate and detailed records of individual progress and any significant developments
- Uphold principles of diversity, equality, and inclusion in all aspects of support delivery
- Remain responsive to the changing needs of individuals and adapt support strategies accordingly
