Job Title: Fylde Coast Rapid Intervention and Treatment Team (RITT) Community Mental Health Nurse Band 6

Hours: 37.5 hours – 1.0 WTE

Fylde Coast RITT is a crisis Older Adults Community Mental Health service based at the Lytham Unit, Lytham Hospital.

We are a 7 day a week service with operating hours from 8am to 8pm, shifts worked at present can be Early (8-4pm) Late (12-8pm), including weekend working. Flexible working can also be considered to work 4 x 9.5 hour days (8am – 6pm / 10am – 8pm daily).

We are a multi- disciplinary team, with close working links with other health departments, social services and voluntary agencies.

Our service users can present with both functional and organic mental health difficulties which are often combined with complex social care and physical health needs.







RITT has 2 core functions:

· Home treatment:

The provision of care coordination for patients who are experiencing acute mental health difficulties and who are presenting as high risk to themselves or others in the community and so require intensive home support from community services. Conducting Gatekeeping assessments for mental health inpatient admissions is a core component of this role as is facilitating early discharge from inpatient units.

· Care Home Liaison Team:

Providing support for service users and carers for people who live in a residential home and are presenting with Behavioural and or Psychological Symptoms of dementia.

We are looking to recruit a Band 6 Community Mental Health who has an enthusiasm for and passion to work within older adult’s mental health services.

The Band 6 Nurse will have the opportunity to consolidate their skills in mental health nursing as well as developing the specialist skills needed in an older adults mental health.

There is an expectation that the successful applicants would be able to act up in the absence of the Deputy Manager and Manager, they would also have some supervision responsibilities to more junior team members.

This post is subject to an enhanced DBS Disclosure and the successful applicant will be required to pay for their DBS check
