Summary


An exciting opportunity has arisen in the Children's and Young Peoples Intensive Support Function Team for an experienced/skilled and passionate Clinical Team Manager.

We are looking to recruit a Clinical Team Manager to lead, manage and support the team in providing high quality care to children, young people and their families. This is an exciting opportunity to be involved in the service development of the intensive support function team, and the post holder will work closely in the Children, Young People and Families (CYP) leadership team as we introduce the service across Halton, Knowsley, St Helens and Warrington.

This post is subject to an enhanced with children’s and adult’s barred list Disclosure and Barring Service (DBS) Check.

The intensive support function team will provide a designated resource to reduce/manage behaviours that challenge, working with the child/young person in their own residence, reducing/preventing the need for the use of restrictive practices, inpatient services, and out-of-area/residential placements. The intensive support function team works within a Positive Behavioural Support and Trauma-Informed Framework. This service will work across Halton, Knowsley, St Helens and Warrington.

Delivery of ISF input is throughfour core functions of support,in collaboration with the lead clinician/care coordinator from existing CYP service provision:
1. Assessment, treatment and support for individuals who display behaviour that challenges
2. Provision of support, and person specific training for other agencies supporting those individuals
3. Supporting transitions from inpatient and other settings
4. Crisis response

The successful candidate will oversee team performance, be responsible for managerial supervision and leadership of the team, and also be responsible for clinical leadership of the team along with clinical decision making for the team.

The post holder will carry managerial responsibility for Children, Young People and Families (CYP) Learning Disabilities/ Austism Spectrum Condition (LD/ ASC) Services within the Community Care Division and be responsible for the overall standards which will include referral management and access, assessment of need and delivery of care within CYP LD/ ASC Services.

The post holder will operate as part of an integrated multi-disciplinary team but be expected to be flexible in terms of travel to undertake duties.

The post-holder carries continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care, and the setting of standards of care within that team.

The post holder will be aligned to CYP Learning Disabilities/ Autism Services and may be required to work flexibly across all divisional sites to provide response to the service needs of the physical and mental health clinical teams providing cover as directed by the Service Manager to ensure continuity of a quality service provision.
