Summary


Network Navigators will be responsible at ward level for ensuring that each patient’s network of care is involved early in admission to inform care planning and timely discharge. They will organise this system around the patient to join key meetings during the admission.



The quality of interaction with patients and those involved in their care will be characterised by an approach that is therapeutic, strengths based, and recovery orientated. The network will include as wide a range of relevant individuals and services as required to support the patient’s recovery. This will include carers, community mental health and substance misuse teams, voluntary sector organisations, criminal justice system staff and housing providers where relevant.

The post holders will work closely with Discharge Coordinators and their designated ward(s) to identify individual patient’s strengths and barriers to discharge at the earliest opportunity following admission. They will work with the clinical teams involved to ensure timely resolution of barriers to ensure patients are discharged when they are clinically ready. Facilitating the contribution of carers is a key activity.

Network Navigators will connect with a wide range of internal and external parties to support care planning and discharge planning. Within the Trust, this will include liaison with other resources that have a focus on discharge including mental health Social Workers, Crisis Resolution Teams, Personality Disorder Liaison Nurses, and Occupational Therapists. External pathways will include interface with general adult social care and a range of housing and other accommodation pathways. Single gender women’s wards will require connection to specific services e.g. domestic violence and refuge services and Children’s Services.



Gathering feedback about the quality of these interactions and interventions will evidence improved patient and carer experience and support continuous improvement. Data entry will be used to demonstrate the expected impact on reduced length of stay in hospital admission duration.

This is a new post which will be reviewed with postholders as part of an evaluation.















1. RELATIONSHIPS/COMMUNICATIONS



The post holder is expected to network with a wide range of internal and external parties across the public, private and voluntary sectors and to promote engagement with carers. There are key internal relationships with inpatient teams and NLMHP community teams.



The post holder is expected to establish and maintain positive interpersonal relationships with other staff members, service users and carers characterised by trust, mutual respect, and open, honest communication.

2. KEY RESPONSIBILITIES





Clinical/Practice



· Actively participating in multi-disciplinary team discussions to identify the needs of patients that will enable sustainable discharge.

· Working directly with patients to establish details of their network of care.

· To communicate with a range of key stakeholders to establish early care planning meetings.

a) Ensuring the initial care planning meeting occurs within the first few days of admission.

b) Actively linking patients and carers in with local services.

c) Referring service users and carers to statutory and third sector organisations.

d) Identifying barriers to discharge and ensuring the network of care is involved in resolution planning.



· To keep the Discharge Intervention Team supervisor informed of key information related to discharge planning.



· To support other members of the team in ensuring key information is recorded in the EPR and the Patient Flow Tool.



· To ensure the ward Occupational Therapist interventions are aligned with the patient’s initial care plan needs and supports timely discharge planning.



· Contributes towards the ongoing assessment of risk issues in relation to the service users’ care plan. Informing the treating team if there are:



a) any changes in the patient’s physical or mental state

b) any changes in social situation

c) any incidents related to risk to others.



· To recognise and respond appropriately to behaviours that challenge in line with Trust policies.



· To assist in the organisation of the day-to-day smooth running of the service, reporting in a timely manner any issues relating to the fabric or condition of the clinical environment.



· To understand and follow the Trust risk assessment policy, including being aware of the need to follow all safeguarding procedures; being aware of the necessity to safeguard and promote the welfare of children, young people and vulnerable adults at all times when working with inpatients.



· To develop and maintain relationships with partner organisations and local and national organisations relevant to patients and carers.



· To have an awareness and understanding of the Mental Health Act (1983) and the Mental Capacity Act (2005).



· To actively contribute to clinical or governance reviews of the teams activity and the monitoring of performance. Contributing to relevant audit practice development on research activity that may be taking place in the work area.



· To contribute as required to reviews of policy and procedures.



· To follow all Trust policy relevant to patient care within the role’s scope of practice.



· To be able to work and travel across a geographical area.









Communication



· To maintain confidentiality of information, in accordance with Trust Policy.



· To contribute to effective communication with patients, carers, family and friends and staff colleagues. Always acting in a dignified and responsible manner with patients, their family, visitors, carers and colleagues; listening carefully and responding using appropriate language and accessible communication methods and skills which acknowledge cultural differences and professional boundaries.



· To maintain timely and accurate patient records under supervision and in line with Trust and service line policy and procedure.



· To communicate appropriate and accurate information to and from other departments and service areas as required.



· To attend and participate in team meetings and contribute ideas to multi-professional team discussions.



· To work in ways that support shared responsibility within the team, demonstrating support, respect, and courtesy towards colleagues.



· To ensure that all complaints or compliments received are dealt with in accordance with Trust policy.



Health and safety



· Contribute to the requirements of the Health and Safety At Work Act 1974.



· To assist in the monitoring and maintenance of a safe environment for patients, visitors, and staff.



· To ensure safe working practices and working environment including monitoring and maintaining cleanliness of the service area.

· To ensure appropriate use, maintenance, cleanliness, replacement and storage of all material and equipment.



· To abide by local and trust policies and procedures with relation to health and safety.



· To be aware of infection control policies, procedures and the importance of protecting all building users in maintaining a clean and healthy environment.







Professional development, education and training



· To be managed by and receive supervision from a nominated line manager, including annual review of performance as part of the appraisal system.



· To attend and participate in regular personal development, ensuring personal compliance with core skills training requirements.



· To attend all core skills training as required, attend appropriate training courses, and keep up to date with developments within the practice area.



· To actively contribute to reviews of the teams activity and the monitoring of performance in line with the Trust directives and audits. To participate in relevant audit or research activity that may be taking place in the clinical area.





Responsibility for Administration



· Contribute to the administrative duties required by the team to ensure the smooth running and efficient running of the clinical environment.



· To create and manage spreadsheets and databases and develop reports for audit purposes and to support the efficient running of the service.



· Ensuring clinical records are kept up to date.



3. GENERAL


• All staff are responsible for compliance with CQC standards.


• The postholder must be aware of, and work in line with, the Trust’s Safeguarding Adults and Children procedures.


4. PERSONAL DEVELOPMENT



· All staff are required to be appraised by their line managers at least once a year at a personal development review meeting where progress made over the last year is discussed and agreed. Focus on the following year’s departmental and personal objectives will be identified, discussed and agreed. Where necessary, help and support will be provided, and development opportunities agreed in line with service provision and knowledge and skills competency framework.
