The post is based in Health and Social Care Partnership Community Care within Montrose House Care Home. Reporting to Unit Manager. You will be involved in providing a range of physical, psychological and social opportunities and direct support to assist service users in achieving their maximum level of independence whilst in a supportive environment. You will work as a member of the team in promoting independence and providing care and support as directed in a support plan under the supervision of senior staff. Assist with personal care routine such as getting out of bed, retiring to bed, washing, dressing, bathing or showering in conjunction with individual support plan. Undertake enhanced personal care tasks when required such as sensory impairment, continence care, catheter care, stoma care, moving and handling using specific aids, adaptations, equipment or hoists. Support service users to maintain an adequate diet and fluid intake such as assisting with choices, maintaining food hygiene, providing supervision at mealtimes, prompting and encouragement, offering snacks and ensuring adequate diet and fluid intake throughout the day. Support service users to experience a wide variety of social and spiritual opportunities based on likes and needs and offering new opportunities. Contribute to assessment and care planning process by reporting back to senior member of staff, completing daily report writing and supporting service users to record their views and wishes in daily report. Attend review meetings where appropriate. Liaise with other individuals or services on behalf of service users and in accordance with their wishes. Work in compliance to National Care Standards.
