Summary


If you are looking to deliver and coordinate relationship-centred dementia care in order to improve the experience of carers/families affected by dementia with complex needs, this is an exciting opportunity to join the Barking & Dagenham Older Adults Mental Health Team and Memory Service. We are seeking someone enthusiastic who understands the challenges faced by families and individuals living with dementia and how these challenges impact their well-being. This is a full-time position, Monday to Friday.

You will be based at the Broad Street Centre in Dagenham and will be part of a well-established multidisciplinary team, including nurses, occupational therapists, psychologists, consultants, and specialty doctors. You will also collaborate with Admiral Nurses working within BHRUT and across the NELFT boroughs.

The Trust and Dementia UK supports ongoing professional development through a range of in-house training and growth opportunities.



To provide specialist dementia nursing expertise and support to those affected by dementia, with a specific focus on family carers, who experience difficulties within a defined locality of Barking & Dagenham.
To provide specialist nursing assessment, evidenced based intervention, practical advice and emotional support for family carers of people with dementia and where appropriate people with dementia themselves, at all stages including bereavement, within London Borough of Barking & Dagenham
To offer expertise and guidance to professional colleagues, the general public and others in supporting family and relationship-centred approaches and best practice in dementia care.
To assist with the development, evaluation and audit of this specialist area of practice and of the Admiral Nursing service.

Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focussing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - Outer London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,313 to a maximum of £5,436 p.a. pro rata for part time).

High Cost Area Supplement - Fringe

This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (with a minimum of £1,192 to a maximum of £2,011 p.a. pro rata for part time).

Leading, delivering and coordinating relationship-centred dementia care within operational areas of Barking & Dagenham, in order to improve the experience of carers/families affected by dementia with complex needs.

Work in line with the Admiral Nurse Competency Framework at an enhanced specialist level, the post holder will provide:

.Specialist bio-psychosocial assessment and support for people with dementia and families with complex needs.

· Facilitation of education and training for staff in the delivery of evidence-based dementia care.

· Clinical leadership with policy development and quality improvements in relation to dementia care.

· Consultancy to other health and social care professionals regarding dementia.

· Leadership within Barking & Dagenham as part of the transformation of dementia care in the borough.
