More detail about the role In line with the Royal Devon University Healthcare NHS Foundation Trust's commitment to timely discharge we are currently recruiting a full-time Discharge Coordinator. The key areas of responsibility will be to support the patients' journey through their Hospital stay to ensure a smooth transition from admission to discharge home or to alternative agreed placement. You will be integral in the daily tactical meetings to support patient flow; sharing information on your areas on expected discharges and progress of patients continuing their journey towards discharge, escalating any blocks to achieve this. You will need to forge close links with the Therapy teams as well as the wider MDT to enable safe discharges and communicate plans regarding discharge with patients and their families from admission. If you have a good understanding of patient admission to discharge pathways, excellent organisational and communication skills, and enjoy working within busy and dynamic teams then this position is for you. Previous experience in a Ward environment or Healthcare is essential. Working Pattern: 37.5 hours per week Interview Date: To be confirmed Pay: The salary advertised for this position reflects the Agenda for Change 2024-5 rates, inclusive of a 5.5% pay award implemented from October 2024. If you join before this date, the pay award will be backdated to April 2024 or your start date, whichever is later. Until then, remuneration will be based on the 2023-4 pay bandings (Pay scales for 2023/24 | NHS Employers).
