Summary
Liaise effectively with members of the relevant multidisciplinary teams.Liaise effectively with managers within clinical directorates.Co-work with Occupational Therapy, Speech and Language Therapy, Nursing, Education and members of other professions.Attend and contribute to relevant meetings as required.Communicate effectively within own service and with other colleagues.In conjunction with colleagues develop and implement the patient pathway within the Therapies, Education Service fitting in with the pathways of the other Clinical Directorates.Be actively involved in Quality Governance and quality assurance activities as required.Accurately record statistical information.Contribute to the Care Programme Approach process.Keep accurate, appropriate and contemporaneous patient records in accordance with the Forensic Information Governance Group and Trust Policy.Support and deliver innovative practice.Undertake administrative tasks as required by the service and the Directorate. Lead on specific developments as required.
