Summary
To deliver a specialist care and support service within West Norfolk including: You will work within the Diabetes Team providing expert knowledge and skills. You will be responsible for managing a caseload of patients with complex diabetes needs, using evidence-based practice and patient centered goals to assess, plan, implement and evaluate interventions. The role will include health education to the service users, their families, carers, and other agencies involved in their care management. You will be required to work independently working in patients homes across your place with access to the diabetes team remotely. Provision of education and support to patients who have diabetes to enable them to self-manage and reduce their risk of complications from diabetes. Delivery of expert care and support in a variety of settings including home visits and telephone support. To provide educational support to professionals running Diabetes clinics in general practice for them to provide patients and their carers with systematic care and support. Deliver a specialist Diabetes Service to community care, clinics, nursing/care homes and housebound patients across the primary/secondary interface which compliments and enhances the existing community based service.
