Summary

Job overview The Elms Medical Centre is a well-established training practice providing patient centred care for our population of 9850 patients. We currently have a fantastic opportunity for aGeneral Practitioner tojoin our experienced and friendly team on a salaried fixed term contract for 12 months. The post holder will support the clinical team to manage a busy and interesting surgery. You will be expected to carry out all the clinical duties and responsibilities of an NHS General Practitioner using the standards expected by the GMC and RCGP in line with appropriate national and/or local guidelines. Salary depending on experience - very competitive 4 sessions per week -Thursday & Friday - start date early December. Main duties of the job The post holder will make themselves available to undertake a variety of duties including: Surgery consultations, telephone consultations and queries, triage, home visits, checking and signing repeat prescriptions. We operate a total triage system and the post holder will be expected to take part in on-call or duty doctor responsibilities. Making professional autonomous decisions in relation to presenting problems, whether self-referred or referred from other healthcare workers within the organisation. Assessing the health care needs of patients with undifferentiated and undiagnosed problems. Screening patients for disease risk factors and early signs of illness. Recording clear and contemporaneous consultation notes to agreed standards. Record all relevant data to aid the practice in meeting its performance targets. Compiling and issuing computer generated acute and repeat prescriptions. Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate. In general, the post holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care. List size approx. 9800 2 GP partners 1 business partner 2 salaried partners 7 Salaried GPs Nursing team Pharmacist care navigation and operations team Care co-ordinator Integrated PCN staff first contact physiotherapists, social prescriber and pharmacy technicians Dynamic and supportive PCN with integrated PCN staff and services including first contact physiotherapists, social prescriber, health and wellbeing coaches, pharmacy team and womens health hub Clinical Responsibilities In accordance with the practice timetable and as agreed, the post holder will make themselves available to undertake a variety of duties including: Surgery consultations, telephone consultations and queries, triage, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion. The post holder will be expected to take part in on-call or duty doctor responsibilities on a rota basis. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other healthcare workers within the organisation. Assessing the health care needs of patients with undifferentiated and undiagnosed problems. Screening patients for disease risk factors and early signs of illness. In consultation with patients and in line with current practice disease management protocols, developing care plans for health. Providing counselling and health education. Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate. Recording clear and contemporaneous consultation notes to agreed standards. Record all relevant data to aid the practice in meeting its performance targets. Collecting data for audit purposes. Compiling and issuing computer generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible). Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate. In general, the post holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care. Administrative and Other Responsibilities To manage the normal administrative workload generated by patient contact such as referrals by e-referrals, letters, medical certificates, queries from other health professionals, third party reports etc. Practice administration which is done on a shared basis e.g. repeat prescriptions, clinical correspondence, results. Awareness of and compliance with all relevant practice policies/guidelines/protocols e.g. prescribing. A commitment to life-long learning and audit to ensure evidence based best practice. Contributing to evaluation/audit and clinical standard setting within the organisation. Contributing to the development of computer-based patient records. Contributing to the summarising of patient records and read coding patient data. Attending training and events organised by the practice or other agencies, where appropriate Experience Desirable criteria Experience of QOF and clinical audit Experience of medicines management Experience of GP triage and working to modern general practice Experience or interests in digital systems Qualifications Essential criteria Fully Qualified GP or due to qualify imminently Full GMC registration National performers list registration
