Summary

Supported Living Lead – Outreach Redcar and Cleveland.

Full time – 39 hours

Competitive salary of £24,726.87

We have an exciting opportunity for a Service Lead to join our team at our supported living services in Outreach Redcar and Cleveland.

Reporting to the Deputy Manager, the purpose of a Service Lead is to lead a cluster of support services that provides agreed level of support to the people we support, in order to achieve their individual goals, and to lead a full and productive lives as valued member of their community. A service lead plays a key role in the assessment of service users and the development and implementation of a Person Centred Plan.

We are looking for someone with creative experience, to act as a role model for our staff teams and take some responsibility for the day to day running of the activities undertaken at the service.

You will be working as a service lead in two supported living homes and small outreach environment supporting adults with Learning Disabilities and Mental Health conditions to maintain independence and gain new skills which enables them to live more independently.

You will be required to undertake specified managerial responsibilities which contribute to the overall management of the support services provided such as completing rotas, supervision of staff, producing medication and finance audits, ensuring that we meet compliance and liaising with families, social workers, and health professionals.

You will also be required to support on shift where needed to meet the needs of the people we support. Some of these duties would include, to plan and prepare meals, keep their home clean and tidy, support with medication if needed, and personal care. You will also be required to update support plans and risk assessments and support people out in the community to access a variety of activities.

You will need to be flexible and be able to work various shift patterns which would include sleepovers. Due to the how remote the area is, a driver is desirable.

It is essential that you have the following experience:-

Demonstrable track record of working with people with learning disabilities in an enabling environment.

Good understanding and commitment to the principles of Person Centred Care.

Understanding of the needs of people of the relevant service use group, e.g. Learning Disabilities, Mental Health, Complex Care, Autism etc.

A Level 3 in Health and Social Care.

Experience in leading a staff team would be an advantage.
