Central London Community Healthcare Trust have an exciting opportunity for a Band 7 Senior Nurse within the Brent Health Matters Clinical Service.

The successful candidate will demonstrate flexibility, excellent communication skills and the ability to work as a team member as well as using your own initiative. This is an excellent opportunity to develop your clinical skills and knowledge alongside service development, within the Core20plus5 framework of long-term conditions.
The post holder will work as a complex case manager, with direct supervision. You will be involved in delivering integration, working with GP’s, Social services and the third sector organisations.
You will be providing case management, clinical staff support, mentoring, clinical assessments for people who have difficulty accessing healthcare services and with some of the social determinants of health inequalities for the most vulnerable communities, supporting the service at GP practices, community outreach events and where appropriate, supporting healthcare closer to home.
Weekend and Evening working will be required as part of the normal working pattern
