Summary

Full time equivalent 40 hrs per week and on-call duties as per the rota needs.

Rate of pay £13.50 per hour plus on call payment of £20 per day (travel time not included).

An exciting opportunity has opened up in our branch based in Gosport for a Care Coordinator to join our team.

Numada Homecare Limited are looking for an enthusiastic, hardworking, motivated, proactive and caring Care Coordinator to join our Homecare team, supporting the registered manager with the day to day running of the domiciliary care service primarily based in Gosport and local surrounding areas.

About the role:
As the Care Coordinator you will manage all aspects of future and day to day planning of our care service as part of our friendly and dynamic care delivery team. You will promote and support the delivery of compassionate care for all of our clients.
You will be assisting and supporting the team to ensure that key business and compliance targets are met. You will strive to deliver a high standard of accuracy in rota management, care assessments, care planning and quality monitoring. You will also have the opportunity to remain ‘hands on’ as care colleague, as required with care visits and assessments.

Key responsibilities of the role will include:
* rota management;
* supervision of staff;
* care assessments and care planning;
* undertaking audits and spots checks as required by the registered manager
delivery care to individual clients.

It is expected the role will aim for a minimum of one day a week providing care and 4 days a week to complete assessments, care planning, scheduling, supervisions, spots checks (this list is not exhaustive).

Weekend and evening work will be required to perform the role. There will be a requirement to provide on-call support at least two weekends per month, with the view that if needed you will provide care to our clients during this time.

Given the nature of the service, you will be required to work a variety of flexible shifts to ensure that the service provided to the public across a 7 day rota framework. This includes evenings, weekends and bank holidays.

The ongoing development of the service requires a flexible workforce to meet client’s needs with the possibility of working at various locations within Gosport, Fareham and the surrounding areas.

Your skills will include:
At least one years' experience in a care coordinator or equivalent role.
Excellent communication skills, both written and oral
Ability to work in a team, remain calm, proactive and professional at all times
Self motivation and excellent organisational skills
Ability to use your own initiative and work under pressure
Ability to prioritise work according to need and urgency
Ability to build rapport with new people
Experience of rostering and using electronic monitoring systems is important, as is a good telephone manner and excellent record keeping skills.

You will have:
At least one years' experience in a care coordinator or equivalent role.
Full UK driving licence with business use insurance
Access to own car
Level 3 in Heath & Social Care (ideally)
Positive attitude

Please note: Successful candidates will be required to undergo an enhanced DBS check and demonstrate a right to work in the UK.
