Summary

Clinical Care Act as Advanced Nurse Practitioner, demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional nursing roles. Undertake consultations with patients as an autonomous practitioner and using own clinical judgement to diagnose, treat or refer patients. Diagnose and manage both acute and chronic conditions, integrating both drug- and nondrug-based treatment methods into a management plan. Make direct referrals to primary, secondary, and social services within locally agreed pathways, guidance, and protocols. Work with patients and their carers in order to support compliance with and adherence to prescribed treatments. Ensure care plans are in place, including emergency health care plans following discussion with the patient and family members. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, within scope of practice. Ensure complete and accurate documentation of each and every patient contact. Work as an autonomous practitioner and as part of multi-disciplinary and multiagency teams in order to ensure patients needs are met. Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care Professional Adhere to the NMC Code of Professional Conduct and always work within the scope of professional practice. Ensure that professional practice adheres to organisational and NMC policies, procedures, and guidelines. Maintain a professional manner and act as a positive role model to other members of the Primary Care team. Maintain confidentiality with regard to information pertaining to patients and staff. Ensure that the service interfaces with all other departments in a professional and productive manner, providing an effective service to partner organisations and other service providers. Where required provide supervision and mentoring to other staff in order to help them develop their knowledge, skills and abilities. Deliver care according to NICE guidelines and evidence-based care. Initiate and participate in the maintenance of quality governance systems and processes across the organisation and its activities This role is for Part/Full Time and salary is dependant on experience.
