Summary
The post holder will: Workas part of the multidisciplinary diabetesteam (MDT)in primary care todevelop atailored management plan foridentified patients with diabetes. Use their expertise to provide evidence-basedprescribing advice to ensureeffective, appropriate, cost-effective and safe use ofmedicines and promote a patient-centred medicines optimisation agenda for patients with diabetes Undertake aclinical medication review for each allocated patient,utilising GP computer systems and discussfindings at the MDTso that the requiredactionscan be built into the care plan. Facilitate good prescribing practice in diabetes across the primary and secondary care interface Deal directly with patients/carers as required, to improve concordance with diabetestreatment in order to achieve better health outcomes. Work with the ICScommissioning team to monitor thedelivery of the diabetes aspects of the primary care contract by practices.
