Job Title:Advanced Clinical Practitioner Team: Clinical Base Location: Local Care Direct Sites Accountable to: Director of Clinical Governance and Quality Line Manager: Named Clinical Services Manager Hours of Work: Various (incl. weekends, evenings and public holidays) Staff Group: All Clinical & Dental Triage Nurses DBS Check Level Required: Enhanced with Child/Adult Barring due to providing care Vaccination Requirement Statement: Covid19 double vaccination not required but recommended. This job has been rated as Medium Risk for requiring Hepatitis B immunisation vaccination is recommended, therefore Post Holders will be offered a Hep B immunisation. Occupational Health clearance required. Flu jab recommended and will be offered. Job Purpose: The Advanced Clinical Practitioner will practice, without direct supervision, as an independent primary / urgent care healthcare practitioner within various clinical environments, participating in care of all age ranges and presenting conditions. The Advanced Clinical Practitioner will undertake assessment, diagnosis, treatment/referral and discharge of patients with primary/urgent care needs within their clinical competence. The Advanced Clinical Practitioner will provide clinical mentorship and leadership to all trainee and peer practitioners within the workplace when required. KEY PRINCIPLES Clinical Independently prioritise, assess, treat, and safely discharge/refer patients attending with undifferentiated and undiagnosed urgent primary health care problems Undertake a range of clinical consultations via a range of modalities primarily face to face but may include telephone and home visits (service specific). Act as a referral agent and care co-ordinator by establishing multi-agency collaboration across the primary/secondary interface to meet the needs of the patient. Work within the parameters of current prescribing legislation or approved Patient Group Directives. Provide professional and clinical advice and direction to colleagues within your team. Maintain accurate, contemporaneous records. Research and Quality Demonstrate evidence-based care, working within agreed guidelines and protocols. Participate in audit, data collection and activity monitoring to improve performance and inform future service developments. Proactivity report patient safety incidents in line with NHSs Just Culture and respond to Local Care Directs Quality Teams requests for information during incident investigations. Take an active role in the management of safety incidents and organisational learning. Management Actively promote integrated working in collaboration with other health care professionals both within and outside the LCD organisation. Contribute to the development, implementation, review and evaluation of clinical protocols. Ensure effective communication, participating in meetings and liaising with other professionals, services and agencies as required. Professional and Educational Develop own skills and competencies through education, training and development activities related to the current and future demands of the role, seeking opportunities to enhance skills, undertake annual internal performance development review (PDR), in order to maintain current Registration and achieve Revalidation standards. Work within codes of conduct and practice set out by relevant professional body. Comply with the corporate governance structure in keeping with the principles and standards set by the organisation. Ensure clinical supervision needs are met. Participate in teaching and mentoring other health professionals including, but not exclusive of, trainee ACP, trainee UCP and GP Registrars from Deanery. COMMUNICATIONS & RELATIONSHIPS To ensure agreed lines of communication and reporting are implemented within the team to support all employees understanding LCD business plans, objectives, priorities and changes. To promote effective cross-functional working and that the operational team supports the achievement of objectives, standards and protocols of other LCD areas. To attend internal and external meetings representing LCD and engaging in discussions appropriate to the role and level of the post. PATIENT CARE & SAFETY To ensure that the quality of patient care and safety is firmly at the centre of the organisations objectives and vision. To ensure the management and monitoring of the highest standards of safety and quality in the delivery of healthcare and related services, and manage risk within Local Care Direct. Recognise and provide appropriate access and intervention so that all patients can receive person centre support, in line with organisational objectives to address/reduce Health Inequalities and promote Health Literacy. FINANCIAL RESOURCES To ensure financial policies and procedures are maintained and the budget is adhered to and that best value for money is achieved. CONFIDENTIALITY To operate and ensure the team operates within information governance protocols and that business confidentiality is maintained at all times. TRAINING To complete the defined company induction programme and core training for this role and mandatory refresher training thereafter and ensure that accurate records of training are maintained.
